School Dist. Student Immunization Record Deadline 2/21/18

In a notice from District Nurse Patricia “Trish” Walker, it’s time to make sure that your student’s immunization record is up-to-date. Important immunization exclusion dates are coming up for the Brookings-Harbor School District.

A report was submitted to the local health department on Jan. 17, 2018, listing students who had an incomplete immunization record. After repeated mailings and notices this fall, our district still had 108 students with incomplete records at the start of January 2018.

The health department will review the report and send out exclusion orders to the parents. These orders will be mailed on February 7, 2018.

The immunization exclusion date for our schools is February 21, 2018. On and after that date, students with incomplete records will be excluded from school until the requirement is met and the school district is provided documentation showing that it has been met.

Immunizations can be obtained at the following places:

School-Based Health Center, 625 Pioneer Road. Hours: 8 a.m. to noon, Tuesday and Wednesday only. You must make appointment
Curry Community Health, 615 5th St. Hours: Fridays only. 8 a.m. to 11 a.m. by appointment only, and 1 to 3 p.m. for walk-ins.

Those who choose not to immunize their students must complete a two-step process.

They must complete an online course and provide the District Nurse with a certificate that is obtained after completing the course, and they need to come to the school and sign the backside of the Certificate of Immunization Status and complete the Non-Medical Exemption portion.

For questions, please contact District Nurse Trish Walker at Kalmiopsis Elementary School (541-469-7417) or via email.
